The NIGTC commentary by Harris (Eerdmans, 2005) gives a lot of helpful insight to this passage on pp.833-846. One quote that I will reproduce is: "On the basis of the formula "heaven and the heaven of heavens" in 1 Kgs. 8:27, the rabbis deduced that there were three heavens, so that the third heaven was the highest heaven ..." (Footnote: The expression is also found in Deut. 10:14, 2 Chr. 2:6; 6:18 and Neh. 9:6.)
